The Supporting Access to Family Planning and Post-Abortion Care in Emergencies (SAFPAC) initiative, led by CARE, used the following health facility assessment and monthly supervision tools during implementation of family planning programs in crisis-affected settings. At project start-up, health facility assessments were conducted to identify gaps in basic infrastructure; the tool was developed in conjunction with staff at Columbia University. The monthly supervision checklist, also co-developed by CARE and Columbia University, allows facility supervision teams to assess general clinic conditions, including infection prevention procedures and supplies, check stock levels, and collect and assess data for consistency across forms, registers, and reports. Readers are free to adapt or uses these tools in their own programs.
